Your First Visit
Your first appointment at Breakwater Oral Maxillofacial Surgery & Dental Implants is a great time to get to know Dr. Larson and our team, and we’ll get to know you and discuss your oral surgery needs. Sometimes surgery can be performed on the same day as your consultation. As a courtesy to you and part of our commitment to keeping you informed, we deliver customized messages throughout each part of your treatment via an easy-to-use messaging platform.

What to Bring
We ask that you bring the following information with you to your first appointment:
- A referral slip and X-rays, if applicable
- List of current medications
- Valid form of ID (e.g., driver’s license)
- Dental/medical insurance information
- Form of payment
Parents of Minors
Patients under the age of 18 must be accompanied by a parent or legal guardian to the consultation appointment.
3D Scanning
We use advanced CBCT imaging technology to capture detailed 3D scans of the head, neck, mouth, and face.
Insurance & Financial Information
Our office accepts many forms of payment and we are in-network with most major insurance companies. We are happy to work with you to help you receive the maximum reimbursement from your insurance, provided we have all the necessary information. The procedures that insurance covers vary with each insurance company. We strongly urge you to contact your insurance provider directly prior to treatment to understand the details of your coverage.
